Problem solving is a part of almost every person’s daily life (both at home and in the workplace). We use problem solving to understand what’s going on in our environment, identify things we want to change and then work out the things that need to be done to create our desired outcome. Root cause analysis is an effective way to systematically prevent and solve underlying issues (instead of just treating ad-hoc symptoms and putting out fires). RCA is the process of discovering the ‘root cause’ of problems to identify appropriate solutions.

When using RCA, a collection of principles can be used to identify the root causes of an event or trend. RCA can show why a system or process failed in the first place.
